RELEASE FORM / WAIVER OF LIABILITY
PLEASE READ CAREFULLY & COMPLETELY

I hereby agree to the following:
I am participating in the Stretching and any other classes offered by Be Flexy Studio Inc during which I will receive information & instruction about stretching, yoga, fitness, etc. I recognize stretching/yoga/fitness etc., requires physical exertion which may be strenuous & am fully aware of the risks & hazards involved. I understand it is my responsibility to consult with a physician prior to & regarding my participation in the classes. I represent and warrant I am physically fit & have no medical condition[s] which would prevent my full participation in the classes.
In consideration of being permitted to participate, I agree to assume full responsibility for ANY risks, injuries or damages, known or unknown which may occur during exercise at Be Flexy Studio Inc (Including, but not limited to, death). I understand that the possibility exists that certain changes may occur during and following exercise. They may include, but are not limited to, muscular sprains, abnormal blood pressure, fainting, disturbances of heart rhythm, heart attack or death. I understand that I can minimize the risk of adverse changes occurring during exercise by adhering to national exercise guidelines adopted by the U.S.
Department of Health & Human Services, which discuss topics such as the importance of warming up and cooling down.

COVID-19 RISK. I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 and any other viruses or infectious diseases by attending classes and events conducted at Be Flexy Studio Inc, regardless of the location of such classes and events, and that such exposure or infection may result in personal injury, illness, permanent disability or death. I understand that the risk of becoming exposed to or infected by COVID-19 or any other viruses or infectious diseases at Be Flexy Studio Inc may result from the actions, omissions or negligence of myself and others participating in activities.

I hereby agree that Kamila Gapizova is not responsible for any accidents that may occur at the Be Flexy Studio Inc.
I hereby agree that I may not file a lawsuit against Be Flexy Studio Inc or any of its employees in the event of accident, injury or death.

CANCELLATION & MAKEUP POLICIES. If you are unable to attend your reservation at Be Flexy Studio Inc, you CAN make it at another day and/or time, BUT you can not have a makeup class after your membership has expired or been extended to the next month and there's no freezes.

Sign up fee (once yearly) is $20.00

REGISTRATION FEES ARE NON-REFUNDABLE!

Cancelletion fee is $10.00

*Personal trainings passes are ONE TIME passes, that can be used only once and have to be used within two weeks from the purchase date. Cancellation policy: $10. NO REFUNDS for no-shows within and/or after two weeks from the purchase date.
